- Company Information (Bill to address, Ship to address, contact person, phone, fax, email).
- Copy of Business/Resale License.
- Credit Reference Sheet if requesting Terms (At least 3 references & 1 bank reference).
- If wishing to use credit card, please attach credit card information.
- Minimum Opening Order of $200
